CCA QR Poster for Clinics
Crohn’s & Colitis Australia (CCA) is undertaking a major 4–year research project in collaboration with Swinburne University of Technology Centre for Global Health and Equity to improve the information and services that are provided to people with Crohn’s disease and ulcerative colitis.
We have developed an A4 poster to display in clinic rooms to help clinicians link their patients to CCA. The resource is free and available to download or order using the link below.
GP Aware
Crohn’s & Colitis Australia (CCA) is partnering with the Gastroenterological Society of Australia, and Australian General Practice Accreditation Limited to support general practitioners and physicians to more effectively participate in IBD management. This project aims to address the third priority area of the IBD National Action Plan – to support GPs to more effectively participate in IBD management.
Crohn’s disease and ulcerative colitis Management Plans
CCA have developed individual care plans for IBD management in primary and specialist care settings. The tool can be used to help improve communication between people living with IBD and their healthcare professional team by storing important information in one document for shared accessibility.
